Yeah, it happens, but the point of the report is that bad treatment is growing, especially with POC.
As for your issue about how the data was collected and if it can be believed; that's how hospitals themselves collect data from patients - by survey. Even our hospital will send home a questionnaire or links for online survey's to fill out after a stay, to gauge how things went. Why should that data be accepted but data bout this kind of survey not be? Anecdotal evidence via survey is all we can hope to get about things like this, and it has to be assumed that the answers are true, even if they may not be.
