I can see where someone would think 'it's not piercing skin, it's not seeing blood, so why not'.

Humans screw up. Their logic fails them and they make stupid mistakes. But, this one at least doesn't sound like some of the other cases, where syringes used directly for injections, have been purposely, deliberately reused to try to 'save money'.

It's nice to see they not only caught it but they owned it and are taking steps despite the 'low' risk.
